On December 18, 2020, the U.S. Food and Drug Administration issued an emergency use authorization (EUA) for the second vaccine for the prevention of coronavirus disease 2019 (COVID-19) caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). The emergency use authorization allows the Moderna COVID-19 Vaccine to be distributed in the U.S for use in individuals 18 years of age and older.

Common Side Effects
The most commonly reported side effects, which typically lasted several days, were pain at the injection site, tiredness, headache, muscle pain, chills, joint pain, swollen lymph nodes in the same arm as the injection, nausea and vomiting, and fever. Of note, more people experienced these side effects after the second dose than after the first dose, so it is important for vaccination providers and recipients to expect that there may be some side effects after either dose, but even more so after the second dose. Learn more.

If you are still interested in receiving the vaccine please complete the following assessment so we can determine eligibility for our upcoming vaccine clinics.  Appointment times can be selected at the end of the screening form.  Appointments must be scheduled at least 36 hours out.

Not everyone will be a candidate for the Vaccine clinic and depending on your responses you may be deferred to an alternate time.

Because this vaccine is being provided by the federal government there are a number of confirmations steps.  This is an initial screening process which will be followed by an additional registration process through NC DHHS COVID Vaccine Managment System (CVMS) 12 hours before your scheduled appointment.  All steps must be followed in order to proceed with the vaccination, if you do not receive the CVMS notification in your email please call 828-894-8271 before you arrive for your appointment.

Lastly, this process is completely optional and you can withdraw this information and request for consideration of the vaccine at any time.  Withdrawal from this process will not affect you in the future should you need care or assistance.
